Please bring these completed forms to the center for faster registration:

• Field Trip Permission Form
• Camper Medication Form
• Bicycle/Walking Permission Form

*Please note: the following camps do not use the standard registration form -- please visit each of these camp facilities directly to register:

• Morningside Summer Art Adventures with Myra Hernandez
• 688 Skateshop II Summer Skate Camp
• Chall Tennis Academy
• In The Zone Hoop Camp
• Triple Threat Basketball Camp with Joe O’Neill

*Please note also: Camps sponsored by JWB (ONLY those indicated as JWB camps below) do not use the standard reqistration form -- please use the JWB Summer Camp Registration Form instead for the following camps:

• JWB Camp at Ross Norton
• JWB Camp Chaka II at North Greenwood

Children should bring their lunch each day. Please, no glass containers. Bring towel and swimsuit
to camps offering swimming.
